Le ligament triangulaire (TFCC) est un des moyens d’union essentiel de l’articulation radio-ulnaire distale. C’est un ménisque biconcave qui présente 2 insertions principales à l’extrémité inférieure de la fossette sigmoïde du radius et à la face inférieure de la tête de l’ulna ainsi que 2 renforcements antérieur et postérieur.

Il existe de nombreuses contraintes au niveau du ligament triangulaire : arrachement, traction sur la base d’insertion interne du TFCC, compression et enfin cisaillements.

Les lésions traumatiques du ligament triangulaire surviennent le plus souvent suite à une chute poignet en pronation ; elles peuvent être isolées ou associées à une fracture du 1/4 distal d’un ou des 2 os de l’avant-bras. Elles ont été démembrées par Palmer.

Le but de ce travail est, après un rappel anatomique et physiologique, de détailler les différentes lésions du TFCC afin d’en dégager les grandes lignes du traitement.

Complex injury of the triangular ligament

The triangular ligament is an essential element uniting the distal radioulnar joint. It is a biconcave meniscus with two principal insertions at the lower end of the sigmoid fossa of the radius and the inferior aspect of the head of the ulna. It also has two anterior and posterior reinforcements.

Several injuries can be observed: pull-out, traction on the base of the medial insertion, compression, and stress tears.

Traumatic lesions of the triangular ligament generally occur during falls on the wrist in the pronation position. Injury can be isolated or associated with fracture of the distal quarter of one of the two forearm bones. Palmer presented a classification.

The purpose of this work was to recall the anatomy and physiology of the different triangular ligament lesions and to present treatments proposed.
