Prevention of postoperative exacerbation of chronic medical disease requires high standard of safety and quality of care in ambulatory surgery and anesthesia.
Age is not a major criterion for selection for ambulatory surgery.
Ambulatory surgery may be considered for patients with ASA score I, II or III with stable medical conditions.
Preoperative medical condition is not a contra-indication to ambulatory surgery, (except severe or end stage) as long as perioperative organization and patient compliance to perioperative instructions are controlled.
In patients with major comorbidity, ambulatory surgery should be considered instead of conventional hospitalization.
Preop treatment must be maintained during the perioperative period. Their interruption does not provide any benefit and could exacerbate an otherwise stable chronic disease.

Les objectifs de la prise en charge des patients en chirurgie ambulatoire sont le maintien du niveau de sécurité et de qualité des soins et le maintien de la stabilité des pathologies en cours.
L’âge n’est pas un facteur majeur dans la décision de prise en charge en ambulatoire.
Les patients ayant un score ASA I, II ou III stable peuvent être prise en charge en ambulatoire.
Aucune comorbidité stable, sauf à un stade très avancé, n’est une contre-indication à la prise en charge en ambulatoire, sous réserve d’une organisation péri-opératoire stricte et d’une observance du patient aux consignes péri-opératoires satisfaisante.
Les comorbidités et traitements lourds stabilisés pourraient être des indications de prise en charge en ambulatoire, permettant de prévenir un déséquilibre des pathologies médicales.
La plupart des traitements sont poursuivis sans interruption durant la période péri-opératoire, leur arrêt temporaire n’apporte pas de bénéfice sur la prise en charge et risque de déséquilibrer une pathologie chronique stable.
