Résumé

Si l’étude de l’anxiété semble avoir connu une éclipse durant les années 1930–1960, les travaux de D. Klein (1964) ont marqué un renouveau d’intérêt pour l’étude de l’anxiété à la fois sur le plan expérimental, pharmacologique, épidémiologique mais aussi clinique. À partir des années 1970, la modification de la classification des troubles anxieux, sous l’impulsion des critères du DSM dans un but de recherches cliniques et pharmacologiques, a permis de distinguer différentes formes d’anxiété dont les origines biologiques semblent mieux établies aujourd’hui au prix d’un démembrement de la classification des névroses. Si la névrose d’angoisse comme entité spécifique a été dissociée en trouble anxieux généralisé et trouble panique, la névrose phobique a été éclatée en phobie sociale, agoraphobie et phobie simple. La névrose de contrainte fait partie désormais des TOC et l’hystérie est devenue un trouble somatoforme. La place de l’angoisse centrale dans la maladie mentale et qui jadis se déclinait sous forme d’angoisse de castration, de séparation ou de morcellement dans le modèle psychodynamique est devenue, sinon un phénomène secondaire, le signe d’un trouble de l’adaptation. L’essor des traitements pharmacologiques de l’anxiété depuis la découverte des benzodiazépines (1957) mais aussi des antidépresseurs nous montre que l’on a changé progressivement de paradigme et il semble désormais qu’à travers une meilleure connaissance des mécanismes neurobiologiques de l’anxiété nous puissions soulager un plus grand nombre de patients, même si dans le même temps nous assistons à une surconsommation d’anxiolytiques. Le paradoxe de l’être humain semble de mieux connaître sa peur pour la maîtriser et sa recherche du bien-être paraît souvent éphémère.

Abstract

If the study of the anxiety seems to have known an eclipse during years 1930–1960, the works of D. Klein [32] (1964) [32] have labeled a renewal of interest in the study of the anxiety at the same time on the experimental, pharmacological, epidemiological but also clinical plan. From the 1970s, the modification of the classification of anxiety disorders at the instigation of the criteria of the DSM in a purpose of clinical researches and pharmacological allowed to distinguish various forms of anxiety the biological origins of which seem better established today at the price of a dismemberment of the classification of the neurosis's. If the anxiety neurosis as the specific entity was separated in generalized anxiety disorder and panic disorder, the phobic disorder was fragmented in social phobia, agoraphobia and simple phobia. The neurosis of constraint is a part from now on obsessive compulsive disorders and the hysteria became a somatoform disorder. The place of the central anxiety in the mental illness and which formerly rolled out in the form of anxiety of castration, separation or division in the psychodynamic model became otherwise a secondary phenomenon, the sign of an adjustment disorder. The development of the pharmacological treatments of the anxiety since the discovery of the benzodiazepines (1957) but also the antidepressants shows us that we changed gradually paradigm and it seems from now on that through a better knowledge of the neurobiological mechanisms of the anxiety we can relieve patients’ largest number even if at the same time we attend an overconsumption of anxiolytics. The paradox of the human being seems to know better its fear to master her (it) and its research for the property–to be seems often short-lived.
