La kératoplastie transfixiante (KT) consiste à remplacer un disque cornéen central de pleine épaisseur. La première indication opératoire est représentée par les œdèmes cornéens. Le rejet est la première cause d'échec de cette intervention. Les principaux facteurs de risque de rejet sont la néovascularisation de la cornée réceptrice et les antécédents de rejet de greffe de cornée. L'astigmatisme postopératoire est le principal facteur limitant la récupération visuelle lorsque le greffon est clair en l'absence de pathologie maculaire. En termes de survie du greffon, les résultats de la KT sont bons que la kératoplastie lamellaire. Un suivi attentif, rapproché et régulier du patient est indispensable au succès de l'intervention à long terme.

Penetrating keratoplasty (PK) consists of replacing a full-thickness central corneal disk. The first surgical indication is corneal edema. Rejection is the leading cause of failure in this procedure. The main risk factors are neovascularization of the receptor cornea and a history of corneal graft rejection. Postoperative astigmatism is the main limiting factor for visual recuperation when the graft remains clear without macular disease. In terms of graft survival, the results of PK are good. Close attention, frequent and regular follow-up of the patient is mandatory to ensure optimal long-term outcomes.
