Les coxites au cours de la maladie périodique (MP) sont rares, elles peuvent être soit propres à cette affection, soit entrer dans le cadre d'un rhumatisme inflammatoire chronique associé à la MP, évoquant habituellement une spondylarthrite ankylosante (SPA).
Nous rapportons dix cas de MP avec coxite confirmée radiologiquement. La MP est isolée dans quatre cas, associée à une SPA dans cinq cas, à une arthrite chronique juvénile (ACJ) dans un cas. Les patients sont répartis en six hommes et quatre femmes, avec un âge moyen de 34,4 ans ± 17,6 (extrêmes 15-70 ans). Dans les cas de MP isolée, la coxite s'est développée à la suite d'accès articulaires aigus à la hanche dans un cas, d'arthrite chronique de la hanche dans un autre cas, et dans deux cas aucune crise articulaire préalable à la hanche n'a été retrouvée. Dans les cas associés à une SPA, l'antigène HLA B27 était absent, dans les deux cas où il a été recherché et dans un autre cas on notait une rectocolite hémorragique sévère. Le recours à la prothèse totale de la hanche ou à une cupule était nécessaire dans six cas dont deux avaient une MP isolée.
Dans la littérature, les hanches avec les genoux constituent 75 % des localisations de l'ensemble des arthrites chroniques de la MP, dont la fréquence est estimée à moins de 5 % des manifestations articulaires de cette affection. L'association MP et SPA (habituellement HLA B27 négatif) est bien connue, mais les liens pathogéniques entre ces deux affections demeurent obscurs.

Hip involvement is uncommon in familial Mediterranean fever (FMF) and can result either from a process specific to this disease or from a coexisting chronic inflammatory joint disease, usually suggestive of ankylosing spondylitis (AS). We report ten cases of FMF with radiologically-documented inflammatory hip disease. Five patients had AS and one had juvenile idiopathic arthritis. There were six men and four women, with a mean age of 34.4 years ± 17.6 (range, 15-70 years). Onset of the inflammatory hip disease occurred after bouts of acute hip symptoms in one of the patients with isolated FMF and after protracted hip arthritis in another; the two other patients had no history of hip symptoms. The HLA-B27 antigen was looked for in two of the five patients with FMF and AS, with negative results in both; another patient in this subgroup had severe ulcerative colitis. Total hip replacement or replacement of the acetabulum was required in six patients, including two with isolated FMF. Chronic joint disease has been estimated to contribute fewer than 5% of the joint manifestations in FMF. In previous studies, the hips and knees were affected in 75% of patients with chronic joint disease related to FMF. The association of FMF and AS (usually without the HLA-B27 antigen) has been well documented, although the pathogenic mechanisms that link these two conditions remain unknown.
