Résumé

La myopie forte se définit au mieux sur le plan biométrique par une longueur axiale (LA) supérieure à 26 mm. Sa prévalence se situe entre 2 % et 4 % chez l'adulte et son incidence est croissante. La physiopathologie de cette affection semble résulter d'une dysrégulation des mécanismes régissant la croissance oculaire. La myopie forte peut être essentielle ou syndromique, évolutive ou non. Cliniquement, elle se manifeste par un aspect albinoïde du fond d'oeil souvent associé à une dysversion papillaire et à un conus myopique. Le staphylome myopique, ectasie sclérale postérieure de localisation variable, s'évalue au mieux par échographie B. Parmi les complications potentielles de la myopie forte, il faut noter la plus grande fréquence de la cataracte et du glaucome qui est de dépistage difficile. La maculopathie myopique comprend la constitution de plages d'atrophie choriorétiniennes, les ruptures de la membrane de Bruch, les complications maculaires précisées par l'optical coherence tomography telles que le fovéoschisis, le décollement fovéolaire, le trou maculaire, la « macula bombée » et les néovaisseaux sous-rétiniens, qui représentent la complication la plus fréquente. Même en l'absence d'autorisation de mise sur le marché, les injections intravitréennes d'anti-vascular endothelial growth factor tendent à remplacer la photothérapie dynamique dans leur traitement compte tenu des bons résultats de nombreuses études pilotes. Le risque de décollement de rétine est également important chez le myope fort. Évalué à 35 % en cas de dégénérescence palissadique, il justifie la surveillance régulière de la périphérie rétinienne en cas de myopie forte avec traitement prophylactique par laser des lésions rhegmatogènes. Ainsi, la myopie forte est une pathologie potentiellement cécitante, justifiant une surveillance régulière afin de prévenir et traiter au mieux ces complications.

Abstract

High myopia is best defined biometrically by axial length (AL) greater than 26 mm. Its prevalence is between 2% and 4% in the adult and its incidence is increasing. The pathophysiology of this disorder seems to result from dysregulation of the mechanisms governing ocular growth. High myopia can be idiopathic or syndromic, evolving or stable. Clinically, it is manifested by an albinic aspect of the fundus often associated with papillary dysversion and a myopic conus. Myopic staphyloma, posterior scleral ectasia with a variable location, is best assessed with B ultrasound. Among the potential complications of high myopia must be noted the greater frequency of cataract and glaucoma, which are difficult to screen. Myopic maculopathy includes the onset of zones of chorioretinal atrophy, ruptures in the Bruch membrane, macular complications demonstrated on optical coherence tomography such as foveoschisis, foveolar detachment, macular hole, bulging macula, and subretinal neovessels, the most frequent complication. Even without market approval, intravitreal injections of antivascular endothelial growth factor tend to replace dynamic phototherapy in treatment of this disorder, given the good results in a number of pilot studies. The risk for retinal detachment is also high in the high myopic patient. Evaluated at 35% in cases of palisade degeneration of retina, it warrants regular monitoring of the retinal periphery in cases of high myopia with prophylactic laser treatment of the rhegmatogenous lesions. High myopia is therefore a potentially blinding disease, calling for regular surveillance to prevent and treat these complications as best possible.
